Consumer vs. Agency Directed Services

At Shades of Life Home Care Services LLC, we are committed to delivering responsive and excellent service to all our customers. We are also proud to offer the following services to our community:   

Agency Directed: In providing Agency Directed Services, Shades of Life is responsible for hiring, scheduling, and maintaining the record of direct support staff in the individual home or the community by providing personal and respite care to the individual and the families. We offer the following resources under our Agency Directed Services:

  • Supportive In-Home- This service is provided for individuals who are Intellectual (ID) or Developmental Disabled (DD) living in their own homes or with family members. A collaborative plan is developed based on the needs of the individual seeking this unique service. We work with adults and children to build the skills they need to become more independent to reach their highest potential within their communities.

  • Personal Care- A personal care aide will provide maintenance services such as shopping, meal preparation, and light housekeeping. Assistance with personal care needs, such as bathing, dressing, feeding, etc. This service is also available for individuals diagnosed with ID, DD, and geriatric.

  • Respite Care- Respite care provides short-term relief for primary caregivers. It can be arranged for just an afternoon or several days or weeks.

 

Consumer Direct Services: Under Consumer Directed Services, Shades of Life Home Care Services LLC (Service Facilitator) provides services to the Employer of Record (EOR). The Employer of Record is responsible for hiring, supervising, and firing the attendants. Wages for this service are paid through DMAS, the fiscal agent.

 

As your Service Facilitators, you can expect the following services and support from Shades of Life Home Care Services LLC:

 

  • Development of your service plan with the Individual and the EOR.

  • Provide the Fiscal Employer Agent with the appropriate forms to establish the EOR.

  • Review this manual with the EOR.

  • Submit your requests for service authorization to the appropriate service authorization contractor.

  • Train the EOR on the required tasks of an employer.

  • Document services as required by the Department of Medical Assistance Services (DMAS).

  • Assists the EOR when completing paperwork for the Fiscal Employer Agent.

  • Conduct reassessment visits.

  • Conduct the annual level of care review process.

 

 

Skilled Nursing- Providing an RN, LPN, or CNA to care for your service needs.
